Reassign an audio recording to the correct patient

If an audio file was recorded on the wrong patient, you can reassign it from Visit Data using the audio toolbar and attach it to the correct Patient and Visit.

Sometimes an audio recording gets captured while the wrong chart is open. Kubo lets you move that audio (and its transcript) to the correct patient and visit in a few clicks.

Reassign an audio file from Visit Data

  1. Open the patient’s Visit Data screen where the audio was recorded.

  2. In the audio toolbar at the bottom, click the duration next to the Record button (for example, 00:52).

  3. In Audio Transcripts, find the audio file you want to move.

  4. Click the three-dot (···) menu on that audio file.

  5. Select Reassign Patient...

  6. In Reassign Audio File, choose:

    • Patient (the correct patient)

    • Visit (the correct visit)

  7. Click Confirm.

⚠️ The destination Visit must already be started for the patient you’re reassigning to. If you don’t see the visit in the Visit dropdown, start that visit first, then return and reassign the audio.

What happens next

  • The audio file and its transcript are attached to the selected patient and visit.

  • Kubo will use the reassigned audio when drafting the clinical note for that visit.

Troubleshooting

  • I can’t find the right visit in the Visit dropdown: Start the visit for that patient first, then try again.

  • I don’t see “Reassign Patient...” You may not have permission. An Admin/Super Admin may be required.

  • I see multiple audio files: Use the timestamp and duration in Audio Transcripts to pick the correct recording.
